Well that should be doable with:

 /etc/init.d/kdm stop; /etc/init.d/kdm start

It's just that that is a bit brutal and you lose all your current
program state.  Personally I prefer to keep all my programs running, so
it would be nicer if there were some magic command line program that
could enable / disable the extra screen.  Something like xrandr or xset
look like the sort of program that _should_ be able to handle it, but
from their man pages they clearly can't :-(
